Immigration Physicals

Zenith Primary & Urgent Care is pleased to provide Immigration Medical Exams (also known as Green Card Medicals) for immigration applicants in Marlborough, MA, and the surrounding Metrowest region. The immigration medical exam is a vital step in green card applications, and our doctor is a designated civil surgeon registered with USCIS to conduct these exams.

What to Expect?

Each immigration physical is conducted in the following order:

First Visit: (Walk-ins are also welcome!)

  • Review of your paperwork and initiation of required medical tests.
  • Collection of necessary documentation and recording it on your I-693 form.
  • Physical examination and administration of any needed immunizations by the Civil Surgeon.

Second Visit:

  • Scheduled approximately 5-10 days after the first visit to allow for lab results.
  • Pick-up of the completed sealed documents and a copy for yourself and your lawyer if needed.

The Physical Examination will be conducted in the first visit itself provided you have all updated documentation of your health records and vaccinations. Please arrive 15 minutes before your appointment to complete the required paperwork. During the exam, the Civil Surgeon will review your health history and conduct a comprehensive physical examination. Female patients will have a female staff member present if the Civil Surgeon is male. The Medical Assistant will then administer any further vaccinations, immunizations, or blood draws as needed.

You will be notified once all reports are received and reviewed, and your documents are ready for pick-up.

What to Bring?

Please ensure you bring the following items to your appointment:

  • Government-issued photo ID (driver’s license or passport). If the exam is for a child, the child will need a photo ID.
  • Vaccine/Immunization records (translated into English if in another language).
  • Hospital or clinic records, including proof of prior treatment for tuberculosis or other infectious diseases.
  • Previous chest x-rays and/or other diagnostic tests.
  • Any mental health records.
  • Immigration Physical Form i693 (you can complete this online or at the clinic. Ensure everything is capitalized and in black ink).
