Annual Physical vs. Medicare Wellness Visit: Which Appointment Do You Need?
An annual physical and Medicare Annual Wellness Visit may sound like the same appointment, but they have different purposes. A routine physical generally focuses on your overall health, medical history, current concerns, and an examination based on your individual needs.
A Medcare Annual Wellness Visit is a prevention-focused appointment designed to create or update a personalized plan to stay healthy and reduce health risks. It is not a routine physical exam.
Understanding the difference can help you schedule the right type of visit and avoid unexpected costs. At Zenith Primary & Urgent Care, we provide adult primary care in Marlborough, MA, and can help patients determine which type of appointment may be appropriate.
What Is an Annual Physical Exam
An annual physical exam is a routine primary care appointment focused on your overall health. During the visit, your clinician may review your medical history, medications, family history, lifestyle, and current health concerns.
The examination itself is based on your individual health needs. Depending on your age, medical history, risk factors, and symptoms, your clinician may check vital signs and perform appropriate parts of a physical examination.
Additional screenings, laboratory tests, or other services may be recommended when appropriate. However, there is not one fixed set of tests that every person receives during an annual physical.
A physical can also be an opportunity to discuss ongoing health concerns, preventive care, chronic conditions, medication management, and any changes in your health.
Insurance coverage for a physical can vary. Your plan may have specific rules about preventive visits, laboratory testing, screenings, and other services, so it is helpful to confirm your benefits before the appointment.
What Is a Medicare Annual Wellness Visit
A Medicare Annual Wellness Visit is a preventive service covered by Medicare Part B for eligible patients. Its purpose is to create or update a personalized prevention plan based on your current health and risk factors.
Unlike a routine physical, the Medicare Annual Wellness Visit is primarily conversation-and prevention-focused. Medicare states that the visit includes a Health Risk Assessment and may involve reviewing your medical and family history, current prescriptions, routine measurements such as height, weight, and blood pressure, health advice, and a personalized plan for recommended screenings and preventive services.
For eligible patients, Medicare Part B covers a yearly Wellness visit. You generally pay nothing for the covered visit when your provider accepts assignment, although additional services performed during the appointment may have separate cost-sharing requirements.
How Is It Different From the Welcome to Medicare Visit
The Welcome to Medicare preventive visit is a separate, one-time preventive appointment available during the first 12 months after you enroll in Medicare Part B.
It focuses on reviewing your medical and social history, preventive services, certain screenings and vaccines, health risks, and developing a preventive care plan. Medicare specifically states that this visit is not a comprehensive physical exam.
After the appropriate eligibility period, eligible patients can receive Medicare’s yearly Wellness Visit. You do not have to have had the Welcome to Medicare visit to qualify for a yearly Wellness Visit.
Because eligibility and timing can depend on your Medicare coverage, speak with your healthcare provider before scheduling.
Annual Physical vs. Wellness Visit

Does Medicare Cover an Annual Physical
If you have Original Medicare, it is important to understand that a yearly Medicare Wellness Visit is not the same as an annual physical.
Medicare Part B covers an eligible patient’s yearly Wellness Visit, but specifically lists routine physical exams among services that Original Medicare does not cover.
This distinction matters when scheduling an appointment. If services beyond the covered preventive benefit are performed, you may have additional cost-sharing. If additional services are not covered under the preventive benefit, you may have to pay coinsurance, meet a deductible, or pay the full amount, depending on the service and circumstances.
Medicare Advantage plans may have different benefits and coverage rules than Original Medicare. Your specific plan should be checked before your appointment.
What Should You Bring to the Appointment?
Whether you are attending a physical or a Medicare Wellness Visit, being prepared can make the appointment more useful.
Consider bringing or having available:
- A current list of prescription medications
- Over-the-counter medicines, vitamins, and supplements
- Your medical and family history
- Immunization records
- Information about previous screenings
- Records from other healthcare providers when relevant
- A list of health concerns or questions
- Your insurance or Medicare information
Medicare specifically recommends bringing medical records, immunization information, family health history, and a current list of medications and supplements to the Welcome to Medicare visit.

Frequently Asked Questions
Is a Medicare wellness visit the same as a physical?
No. A Medicare Annual Wellness Visit is a preventive planning visit and is not a routine physical exam. Medicare.gov specifically distinguishes the two.
Does a wellness visit include blood work?
Not automatically. A Wellness Visit may result in recommendations for tests or screenings based on your health and risk factors, but laboratory testing is not automatically included in every Wellness Visit. Coverage for additional services can vary.
Can a physical and wellness visit happen on the same day?
It may be possible to receive additional services during the same appointment, but they may be subject to separate coverage and billing rules. Medicare.gov notes that additional services provided during a Wellness Visit can result in coinsurance, deductible requirements, or other costs depending on the service.
Ask your healthcare provider and insurance plan about the specific services you need before the appointment.
How often am I eligible for a wellness visit?
For eligible patients, Medicare Part B covers a yearly Wellness Visit. Your first annual Wellness Visit cannot occur within 12 months of enrolling in Part B or having a Welcome to Medicare preventive visit.
What should I do if I am unsure which visit my insurance covers?
Contact your insurance plan or the primary care office before scheduling. Ask whether you are eligible for a Medicare Wellness Visit, whether the provider participates with your plan, and whether any requested services could result in additional costs.
